I expressly understand and agree to be solely responsible for any cost arising out of any bodily injury or property damage sustained through participation in the Blessed Sacrament’s Athletic Camp.

In the event that my son/daughter is injured or become ill, I grant my permission on our family’s behalf in case of emergency that the Blessed Sacrament Athletic Camp personnel may seek medical assistance as may be deemed necessary under the circumstances.  I also certify that he/she is physically fit for strenuous activity according to our family physician.

Basketball Camp Fee:  $50.00

Athletes may participate in one or both camps.  Fee includes T-shirt and instruction.  All proceeds will go to the purchase of uniforms.
